Camp Sdei Chemed pays for an addition Israeli insurance to cover all our campers. This is included in the camp price.Instructions on filling out medical form. Please read carefully.
1. Print out this form.
2. Sign and date the form.
3. Leave blank departure date.
3. Under the signature print your child's name, passport number and date of birth.
4. Fax the second page (the one with the signature) to us at 347- 823-1183.
5. If you have a previous medical condition you must also fill out this form and fax to us at 34-7-823-1183.
